How much incentive can I earn?
Up to $100 for you and another $100 for your spouse. You must complete the health assessment to be eligible for the maximum $100 incentive. If you choose not to complete the health assessment, the maximum award is $50.
When will the incentive be in my pay check?
Incentives are paid out once per quarter. If you have questions regarding when your incentive will be on your paycheck, please call APS Healthcare at 1-866-272-5507.
I have already received $100. Am I able to earn more?
The incentive program re-started July 1, 2010. Effective that date, you may start earning incentives again. For instance, if you took the health assessment in December 2009 and earned $50, you can re-take it beginning July 1, 2010 and be eligible for another $50 incentive. The deadline for completing the health assessment for FY11 is November 30, 2010.
My spouse and I both work for the state. Will we both receive incentives in our pay checks?
No. The incentives for both of you are paid in the pay check of the person who carries the health care.
How do I apply for the incentive?
No need to apply. The TCLW provider will process this for you.
What if I end my employment with the state?
You must be receiving a pay check in order to receive an incentive award, even if you were receiving a check at the time you participated in TCLW.
I have questions about my incentive payment. What should I do?
All health plan members call 1-866-272-5507, option 4 to talk to APS Healthcare. Incentives for FY08 and FY09 are no longer being researched. The deadline for researching FY10 incentives will be December 31, 2010.
